This website is operated by Lopez Dental Center, a dental office at 2716 N Broadway #211, Los Angeles, CA 90031. In this policy, "we", "us" and "our" mean Lopez Dental Center, and "website" means this site and all of its pages.
2. What this policy covers
This policy covers information collected through this website only. Health records created when you become a patient are handled separately, under the privacy rules that apply to dental offices (including HIPAA), and are described in the notice of privacy practices available at our office.

Information you give us. When you send an appointment request, we collect what you type into the form:
- Your name, phone number and email address
- Your preferred appointment date and the reason for your appointment
- Any message you choose to add
Information collected automatically. When a request is sent, we record the date and time and the IP address it came from. Like most websites, the server that hosts this site may also keep standard technical logs, such as the pages requested and the type of browser used.
We do not run advertising or analytics trackers on this website.

6. Please do not send sensitive health details
The appointment form is for scheduling only. Please do not include detailed medical history, insurance ID numbers, Social Security numbers or other sensitive information in it. We will collect what we need securely at the office or by phone.
7. Cookies
This website uses one essential session cookie. It keeps the appointment form secure and remembers what you typed if the form needs to be corrected. It is deleted when you close your browser and is not used to track you across other websites. You can block cookies in your browser settings, but the appointment form may then not work.
8. Third-party services
Some parts of this website are loaded from other companies, which may receive your IP address and browser details when the page loads:
- Google Fonts, for the typefaces used on the site
- Google Maps, for the maps on the home and Contact Us pages
Links to Google and Instagram open those services directly. Their own privacy policies apply once you leave this website, and we are not responsible for how they handle your information.
9. How we protect your information
Appointment requests are stored in a restricted area of the website that is not open to the public, and access is limited to the people who need it. No method of sending or storing information online is completely secure, so we cannot guarantee absolute security.
10. How long we keep it
We keep appointment requests only as long as needed to schedule and follow up on your appointment, and for as long as the law requires us to keep records. After that, they are deleted.
11. Your choices and rights
You can ask us to:
- Tell you what information you sent us through this website
- Correct information that is wrong
- Delete your information, where the law allows

12. Children
This website is intended for adults. We do not knowingly collect information from children under 13 through this website. A parent or guardian should send appointment requests on behalf of a child.
